This form is for requesting medical help and the same form is completed whether you prefer to visit or telephone the Practice. You do not need to complete the form for prebooked nurse appointments like wound care, immunisations blood tests etc.

If you need medical help now
Call 111
If you need medical help now, use NHS 111 online or call 111.
NHS 111 online is for people aged 5 and over. Call 111 if you need help for a child under 5.
Available 24 hours a day 7 days a week.
Call 999
Call 999 in a medical or mental health emergency. This is when someone is seriously ill or injured and their life is at risk.
Medical emergencies can include:
- loss of consciousness
- an acute confused state
- fits that are not stopping
- chest pain
- breathing difficulties
- severe bleeding that cannot be stopped
- severe allergic reactions
- severe burns or scalds
Patient Phone calls
Please note if your appointment is a phone call then our team will try their best to contact you and if the first call is not answered they will wait about 20 mins and then call you a second time. If there is no answer the appointed will be noted as a did not attend and the clinician will have to move on to their next patient.
