Dear Patient, 

NHS National Diabetes Prevention Programme (ICS Health & Wellbeing) ‘Healthier You’

Our records indicate that your most recent blood test shows that your blood results fall into a ‘pre-diabetes’ range.  This increases your risk of developing diabetes in the future.

The Government has implemented the National Diabetes Prevention Programme – ‘Healthier You’. The aim of the programme is to support people, identified as being at increased risk of diabetes, to reduce their risk of developing the disease by providing advice on diet, weight and activity.

We would like to offer you the opportunity to participate and therefore benefit from this programme.

Once referred you will be contacted by a member of the ‘Healthier You’ team who will provide more information and be able to answer any questions you may have.  If you have already been referred to, or attended, the ‘Walking Away from Diabetes’ session you will still benefit from referral to the ‘Healthier You’ Programme.

If you do not wish to be attend this programme at this stage please decline the invitation when contacted.
