Continence self referral

Please see the Continence Service Self-referral form. Patients and/or carers need to return the form to the continence service at csh.spareferrals@nhs.net.

Please do not return the form to our Practice as it will cause confusion. Any patients/carers who would like to enquire about accessing the continence service can simply contact the service by phone (0203 726 0333) or best by email.
